The main characteristic of chronic myeloid leukemia (CML) is the Philadephia chromosome (Ph) formed by translocation of chromosomes 9 and 22. However, 5-8% of patients have a variant translocation involving one or more different chromosomes. In this case, a blood count for a 56-year-old patient showed low levels of hemoglobin and platelets with high leukocytes and further hematological examination diagnosed CML. Cytogenetic analysis of bone marrow (BM) showed a complex four-way karyogram 46,XX,t(1,9,15,22)(p13;q34;q12,q11.2) and Ph positive chromosome, which we present in this study.
